Zero Defects
Zero Defects is Philip Crosby's quality doctrine that defects are a management problem, not a worker problem: the standard is doing it right the first time, paid for by prevention rather than caught by inspection.

- Rework and scrap are eating margin and everyone treats it as normal.

How to run it
- Get visible management commitment: zero defects is a standard, not a slogan from HR.
- Pull together a small quality improvement team with people from every function, not just quality control.
- Measure where quality genuinely stands right now, so you've got a real baseline before you start.
- Work out what poor quality is actually costing you - scrap, rework, warranty, not just reject counts.
- Build awareness across the business: make sure people know what errors cost, not just that they're 'bad'.
- Fix the causes you find as you find them, rather than filing them for a future project.
- Set up a small committee to plan how the zero defects standard gets rolled out and kept alive.
- Train supervisors so they can explain and lead the standard, not just enforce it.
- Hold a Zero Defects Day: launch the standard publicly, in one moment everyone remembers.
- Get individuals and teams to set their own specific, measurable improvement goals.
- Ask people doing the work what stops them getting it right first time, and go find those causes.
- Recognise people who meet the standard - recognition, not bonuses, is the point.
- Bring supervisors and quality people together on a fixed cadence to keep the programme alive.
- Repeat the whole cycle every 12-18 months so the standard doesn't quietly slip back.
A worked example
Situation. Mikko Virtanen ran quality at Tampere Precision, a precision-parts manufacturer supplying automotive clients from Tampere, Finland, where a final-inspection team was catching and reworking about one part in twelve.
Applied. Instead of adding a second inspector, he traced the rejects to a single mis-calibrated stamping die, made 'do it right the first time' the stated standard on the floor rather than 'catch it at the end', and asked machine operators directly what made the die drift.
Result. The die was recalibrated on a fixed schedule instead of a reactive one, the reject rate dropped by roughly two-thirds within a quarter, and the second inspection shift became unnecessary.
The catch
Crosby's standard rests on 'do it right first time', which can tip into blaming individuals for defects actually caused by bad tooling, bad specs, or bad process design upstream. Zero defects as a slogan without the prevention machinery behind it just adds pressure and hides errors instead of removing them. Some environments genuinely need controlled variability, and a zero-defects mindset can choke experimentation in a young or fast-changing operation.
If the fix is 'try harder', you haven't found the cause - Crosby's method fixes the process, not the person.
